So, you've gone through the whole Appeals process, except for the last step, U.S. Federal District Court. This is a civil court and there is a filing fee.
When your letter comes in the mail notifying you that your claim has been denied, you have 60 days from 5 days of the date on the letter. The Court reviews the evidence of your claim and the final decision of your appeal. The Court may send your case back to the Administrative Law Judge to review once more, or they may also direct the Administration to award your benefits.
You will file in the judicial district where you live or where your principal place of business is. If however, you do not live in a judicial district or you do not have a principal place of business, then you get to file in Washington, DC. There are specific rules to follow when submitting your medical evidence and file to the Court. Your lawyer will be able to assist you with this.
